Your feedback is valuable for the continuous improvement of the Sally Letson Symposium and is essential in developing a program that is relevant to your learning needs. Please complete an evaluation for each session that you attended to claim your CPD credits.

The online evaluation will be available on our website. The website will be sent to you after DAY 1 of the symposium.

  1. Login with the email address used during registration (if unknown, contact dnieuwkoop@cos-sco.ca).
  2. Enter your Royal College ID number to facilitate credit transfer to your MAINPORT holding area.
  3. Evaluate and claim credit hours for the sessions you attended.
  4. After evaluating all of the attended sessions, download, print or email a copy of your certificate of attendance.
