Outcomes of Combined Intraocular Lens Exchange Using Intrascleral haptic fixation (Yamane technique) and DSAEK

Purpose: To describe the clinical outcomes of combined IOL exchange and DSAEK using the Yamane technique.
Study Design: Retrospective study
Methods: Included were patients with pseudophakic bullous keratopathy (PBK) and dislocated or anterior-chamber IOL that underwent IOL exchange using Yamane technique combined with suture pull-through DSAEK. Surgery was performed by a single surgeon. Corrected distance visual acuity (CDVA), graft survival, complications and need for any post-operative intervention, were evaluated at 6 months.
Results: Sixteen eyes of 16 patients were included. Average age was 68±12.27 years. Five out of the 16 patients (31.25%) had CDVA better than 20/400 at presentation. The median CDVA improved from 20/400 to 20/100 six months after surgery. Rebubbling was performed in 3 patients (18.75%), 2 patients (12.50%) had graft failure and 2patients (12.50%) developed persistent elevated IOP >24 mmHg and needed topical treatment.
Conclusions: IOL exchange using Yamane technique with DSAEK was shown to be a safe and effective method for visual rehabilitation in patients with PBK and dislocated or anterior-chamber IOL.
